They also have issues with the body they were given, with their physical appearance and the effort they put in to gain just a little weight that doesn't produce any results. Commenting on their weight is just as rude as commenting on the weight of overweight people.
If some stranger comments on someone's weight, usually, people don't get insulted. But if that does a close person, it is a great insult.
